Influence of crank rate in hand cycling.

PURPOSE: The purpose of this study was to determine gross mechanical efficiency (ME) at different power output (PO) levels of synchronous hand cycling and to evaluate the influence of increasing PO by changing crank rate or resistance in medicare power wheelchairs and power wheelchairs users with experience in hand cycling. METHODS: Nine male participants with spinal cord injury randomly performed three maximal incremental hand cycling tests using a computer controlled cycle ergometer. Each test started at a PO level of 50 W with increments of 10 W. In the velocity protocol, PO was increased via crank rate while resistance was constant (VEL). In the resistance protocol PO was increased via resistance while crank rate was constant (RES). In the freely chosen frequency protocol, the participants could freely select their crank rate while resistance was automatically adjusted to obtain the desired PO (FCF). RESULTS: Peak physiological responses were similar in all three tests, whereas PO max was lower in VEL compared with RES and FCF. Similar values for gross ME were found in both RES and FCF protocols, although systematically higher and increasing crank rates were adopted throughout FCF. Nevertheless, differences in gross ME at comparable relative (RES > VEL at 60 and 80% of PO range: 14.09 and 14.40% vs 13.02 and 13.11%, respectively) and absolute (RES, FCF > VEL at 90 W: 14.47, 14.47, and 13.43%, respectively) PO levels were demonstrated. CONCLUSION: These results suggest that during synchronous hand cycling the freely chosen crank rate is not necessarily the most economical, that high crank rates result in a lower ME at a given PO and that freely chosen crank rates increase with increasing PO levels.

Demographic and socioeconomic factors associated with disparity in wheelchair customizability among people with traumatic spinal cord injury

OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and handicap scooters. CONCLUSIONS: The standard of care for manual wheelchair users with SCI is a lightweight and customizable wheelchair. The standard of care for power wheelchairs users has programmable controls. Unfortunately, socioeconomically disadvantaged people were less likely to receive customizable wheelchairs.

In 1999, approximately 6.3 million adults3.1% of the Nation’s adult populationwere under correctional supervision (that is, incarceration, probation, or parole).1 Additionally, 98,913 juveniles (9% of whom were drug offenders) were incarcerated in public or private juvenile facilities for nonstatus offenses.2 Drug offenders accounted for 21% (236,800) of the State prison population in 1998, up from 6% (19,000) in 1980,3 and 59% (55,984) of the Federal prison population in 1998, up from 25% (4,749) in 1980.4 Also, in 1998, an estimated 26% (152,000) of all inmates under local supervision were incarcerated for drug offenses.5 This increase in the drug offender prison population mirrors the steady increase in arrests for drug offenses. The Federal Bureau of Investigation (FBI) reported 580,900 arrests for drug offenses (5.6% of all arrests) in 1980. The number of arrests peaked at 1,559,100 (10.4% of all arrests) in 1997. In 1999, there were 1,532,200 drug arrests, which accounted for 10.9% of all arrests.

In 1997, 82,646 and 993,365 male inmates were in Federal and State prisons, respectively, and 6,426 and 66,242 female inmates were in Federal and State prisons, respectively. Women in State prisons were more likely to report using drugs in the month before their offense (62% versus 56%). Women in State prisons (40%) were also more likely then male inmates (32%) to have committed their offense under the influence of drugs. In Federal prisons, male inmates were more likely than female inmates to report regular drug use and drug use a month before their offense. The percentageage cost per year to incarcerate an inmate of male (23%) and female (19%) Federal prison inmates reporting drug use at the time of their offense was about equal.7 In 1998, 65.5% of convicted jail inmates had used drugs regularly and 16% had committed their offense to get money for drugs. More than one-third (35.6%) of jail inmates had committed their offense under the influence of drugs.8

BJS reports that 24% of the adults on probation in 1998 were sentenced for a drug offense, up from 21% in 1995. In 1995, almost all probationers had one or more special conditions to their probation (such as fees, fines, drug testing, drug or alcohol alcohol drug rehab malibu, or community service). Of those adults sentenced with special conditions, 41% were required to undergo drug or alcohol alcohol drug treatment malibu and approximately 33% were subject to mandatory drug testing.9

OBJECTIVE: To study the longitudinal relation between physical capacity and wheelchair skill performance in subjects with spinal cord injury (SCI) during rehabilitation. DESIGN: Observational prospective cohort study. Measurements were taken 3 times during subjects’ rehabilitation. SETTING: Eight rehabilitation centers in the Netherlands. PARTICIPANTS: Ninety-seven subjects with SCI. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Parameters of physical capacity were upper-extremity muscle strength (manual muscle test MMT sum score), peak oxygen uptake, and peak power output (POpeak). The Wheelchair Circuit consists of 8 wheelchair skills and results in 3 test scores: ability, performance time, and physical strain. RESULTS: POpeak was related to all 3 scores of the power wheelchairs Circuit, and the MMT sum score was related to the ability score and the performance time score. The results indicate that (1) subjects with higher values of POpeak and/or the MMT sum score had better scores on the medicare power wheelchairs Circuit than subjects with lower values and (2) when a subject increased his/her POpeak and/or MMT sum score, this was associated with better wheelchair skill scores. CONCLUSIONS: There is a significant longitudinal relation between wheelchair skill performance and POpeak and MMT sum score during the rehabilitation of people with SCI. To optimize wheelchair skill performance, attention should be directed toward manual wheelchair exercise training and strength training of the upper body.

Perceptions of power mobility use and safety within residential facilities

BACKGROUND: power wheelchairs and medical scooters enhance quality of life by enabling occupation, improving self-esteem and facilitating social interaction. Despite these benefits, the risks associated with power mobility use raise serious concerns in residential facilities. PURPOSE: As there is no gold standard to assess when a client is unsafe, a two-phase study was conducted to develop client-centred guidelines for power mobility use. METHOD: In the first phase of the study, presented here, 18 in-depth, qualitative interviews were conducted with a variety of stakeholders, including power mobility users, other residents, staff and family members. RESULTS: A thematic analysis of the interviews revealed four main themes: 1) the meaning of power mobility, 2) learning the rules of the road, 3) red flags: concerns about safety, and 4) solutions. PRACTICE IMPLICATIONS: Given the importance of power mobility, safety measures need to address issues of mobility and safety for power mobility drivers and those around them.

Meth Abuse in Pregnant Women

Methamphetamine abuse or dependence is not a homogeneous disease. There are many sub-populations who require specialized outpatient drug rehab los angeles, such as the pregnant woman. We have got to find ways to accelerate the process of brain recovery for the heavy abuser.

Soon I will meet with the director of medications development division at the National Institute on Drug Abuse. We will discuss a collaborative arrangement to study a new, nontoxic drug that, in a pilot study, actually improves brain function and fixes brain membranes. This is the kind of medication we could give to pregnant women and to their children who are dependent.

Our Hawaii “ice storm” is now a decade old and shows no clear sign of ending. Our 4-year-old managed care dual-diagnosis program called Community Care Service is working on this disaster. It also adds cognitive “restructuring”, provides transportation to appointments, and monitors via home visits and intensive in-community outreach. Dr. Charles Bogdahn and I are doing a small research study on recovering ice hallucinators in the drug court system.
